You cannot disable FXAA while using Extra High post-processing through the menu. It requires editing gr_init_dx11.lua with MGSV_QAR_Tool — changing the ExtraHigh EnableFilter value from 1 to 0.

Remove third-party frame limiters while testing

MGSV targets 60 FPS, and forced high frame rates from driver panels or RTSS can cause odd behaviour. Restore the game profile to default and apply only a simple 60 cap if needed.

Launch options for display crashes

Test -windowed, -noborder and -w 1280 -h 720 one at a time. Do not stack them — you will not know which one helped.
